Clinically Validated Proof That Your Care Model is Working

Published on May 25, 2026

Patient-reported outcomes matter, but they're hard to capture consistently between visits. Wellness score gives you an easy way to change that — built on PROMIS-10, a validated measure developed by the NIH and used across clinical research and practice.

Patients complete a short monthly check-in on their physical and mental health. Results are scored, trended, and available in the patient profile — ready when you need them.

Instead of relying on fragmented or one-time conversations, you'll see:

  • A standardized wellness score based on PROMIS-10, updated monthly
  • Physical Health and Mental Health scores with population benchmarks
  • A longitudinal graph tracking improvement, stability, or decline
  • Patient-reported context alongside labs and other health data

All within your patient's profile.

What’s new: Wellness score in Fullscript

Wellness score utilizes PROMIS-10 – a patient-reported outcome measure developed and validated by the NIH, used across clinical research and practice. Through a short monthly check-in, patients report on their physical and mental health. Results are captured, scored, and trended automatically.

Learn more about PROMIS-10

This release helps providers:

  • Demonstrate that your approach is working: Track validated patient outcomes over months and years. When a patient's T-scores improve, that's not a feeling — it's a measurable, standardized result you can point to.
  • See how patients are doing between visits: Ongoing visibility into patients’ mental and physical well-being, so you’re not limited to in-visit conversations.
  • Capture outcomes without adding to your workflow: Fullscript prompts patients monthly with a two-minute check-in. Results flow into the patient profile automatically — no follow-up, no manual tracking.
  • Identify changes earlier and adjust care accordingly: PROMIS-10 has established thresholds for clinically meaningful change, so you can distinguish a real decline from normal fluctuation and decide when to check in sooner.
  • Add meaningful context to clinical data: Wellness score provides subjective insights that complement labs and wearable data, helping you better understand how patients are responding to care over time.

How it works

  1. Patients complete a monthly check-in: Patients are prompted in the mobile app to complete a short survey focused on mental and physical well-being.
  2. Responses are standardized into a wellness score: Each check-in updates a simple, clinically validated score that reflects how the patient is feeling.
  3. You see trends directly in the patient profile: Track changes over time with a clear, longitudinal view of patient-reported outcomes.
  4. Patients engage on their own schedule: Patients can complete check-ins at their own pace, with prompts sent every 30 days.

The bottom line

Wellness score brings consistent, patient-reported outcomes into your workflow – so you’re not relying on snapshots or memory to understand how your patients are doing.

With better visibility into how patients feel over time, you can deliver more personalized care, respond to changes earlier, and support better outcomes between visits.
