1. Sign the Elation Authorization & Consent form
Sign the Elation Authorization & Consent form to authorize the Fullscript + Elation integration.
Before signing this form please be sure to you have a Fullscript account. If you do not have a Fullscript account, click here to create your free account.
Fullscript will acknowledge receipt of the form and inform Elation to authorize the application on your Elation account.
2. Sign Business Associate Agreement
You will need a Business Associate Agreement (BAA) signed with Fullscript before you can build treatment plans with the integration. Keep a look out in your inbox for this agreement that will come via DocuSIgn for you to execute.
3. Navigate to a patient’s chart in Elation
Once enabled, navigate to a patient’s chart in Elation.
Then click Fullscript on the left-hand navigation.
4. Begin writing recommendations
Simply sign in using your Fullscript credentials to begin writing a recommendation.
Frequently asked questions
What is the Elation Authorization & Consent (A&C) form and how do I submit it?
The Authorization and Consent form is a requirement from Elation to enable a 3rd-party integration such as Fullscript. Simply review and complete it here and wait for the Fullscript team to let you know that the integration is enabled. This Fullscript team will send an email confirming receipt and a follow up email once approved and the integration is enabled on the Elation end. Once enabled, the Fullscript app will be available under patient information in the patient chart in Elation. You will need to have a Fullscript account and execute a Business Associate Agreement (BAA) in order to use the application.
Why do I need a Business Associate Agreement (BAA) with Fullscript to enable the Elation integration?
All Elation marketplace partners are required to execute BAA with accounts wanting to access their integrations. We have a standard BAA we can share with your team.We will follow up by providing a BAA via DocuSign for easy execution once you’ve completed the A&C form.
Where do I find my Elation Practice Name needed in order to submit the Authorization and Consent form?

Will details of existing treatment plans created in the Fullscript web app be synced to Elation?
No. Only the details of treatment plans that are created using the integration will automatically be synced to the patient chart in Elation.
Where do I access the Fullscript integration within Elation?
Once enabled, navigate to a patient’s chart and click on Fullscript on the left hand navigation, under patient information. Once you have sent a recommendation it will automatically be documented under Chronological Record.
